Patient Process

1. Request an      
Appointment

Request an appointment by calling us at 863 399-2121 and then register your insurance with us if applicable.

 

2. Complete the Intake Forms

We will send you intake forms for your completion. Simply complete the forms before your appointment. 

3. Arrive for your  Appointment

                 Arrive to your scheduled telehealth (online)                          appointment, provide payment for the                                  session, and then discuss treatment                                    options based on your needs. 

 

              4. Treatment and Follow-up

                 Afterward, begin your individualized                                      treatment plan and participate with                                        follow-up as scheduled thereafter. 

5. Insurance               Reimbursement Request

For those patients who self-pay (without insurance), we provide superbills to allow them to easily submit their individual claims to their insurance company to request reimbursement. Superbills present the data from a patient visit needed to submit an insurance claim successfully. Simply complete your individual insurance claim form and submit it to your insurance company along with the superbill receipt from Owens Healing Journey Psychiatry to request reimbursement of the service.
